What to Expect at Your First Visit

How to Prepare for Your Appointment
Before you arrive:
Wear comfortable clothing you can move in
Bring your insurance card and photo ID
Arrive 10–15 minutes early if you haven’t completed forms ahead of time
Patient Forms
To make the most of your time in the clinic, new patients are asked to complete a few forms before their first visit. Forms are typically sent by email after scheduling your appointment, but you may also fill them out in the clinic prior to your first appointment.
